Pennsylvania is one of the largest Medicare Advantage markets in the country, with roughly 1.6 million of the state’s 2.8 million Medicare beneficiaries enrolled in an Advantage plan. That is close to 58 percent, above the national average, and it means most Pennsylvanians reaching 65 are choosing a Medicare Advantage plan rather than Original Medicare.

It is also a state where the plan you can get depends heavily on which side of it you live on. Pittsburgh and western Pennsylvania are dominated by regional systems that own both the insurer and the hospitals. Philadelphia and the southeast are a different market with different carriers. Central and northeastern counties are different again. A plan that is excellent in Allegheny County may not be sold at all in Lackawanna.

What Is Changing for 2027

CMS finalized the 2027 Medicare Advantage rules on April 6, 2026. These apply in Pennsylvania exactly as they do everywhere else.

Change What it means
Plans are paid 2.48% more CMS is sending over $13 billion more to Medicare Advantage plans in 2027 than in 2026. After two years of benefit trimming, that reduces the pressure to cut, without guaranteeing anything in any particular county.
Carrier exits continue Humana is discontinuing plans covering roughly 600,000 members nationally, with non-renewal letters dated October 2, 2026. Whether Pennsylvania counties are affected will not be clear until plan data publishes.
Chronic-illness benefits tighten You can no longer self-certify for special supplemental benefits. Plans must set objective eligibility criteria and publish them.
Debit card rules Plans offering benefit debit cards must disclose every condition and eligible item, and provide a reimbursement alternative if the card fails at the register.
Star Ratings method Largely unchanged for 2027. The 11 measure removals do not begin until the 2028 and 2029 ratings.

What You Will Pay in 2027

Many Pennsylvania Medicare Advantage plans carry a $0 monthly premium, and that is likely to remain common in 2027. The costs below apply regardless of which plan you choose, and they are already final.

You pay the Part B premium no matter which plan you pick. A $0 premium Medicare Advantage plan means $0 to the insurer, not $0 to Medicare. The Part D deductible and out-of-pocket cap apply to the drug coverage built into a Medicare Advantage plan as well as to standalone drug plans, and your plan may set a lower drug deductible than $700 but cannot set a higher one.

More than most states, where you live in Pennsylvania determines which carriers compete for you and how integrated your plan is with your hospital system.

Western Pennsylvania

Pittsburgh and the surrounding counties are shaped by health systems that own their own insurance arms, so the plan you choose can decide which hospital network you use. National carriers compete here too, but the integrated regional plans are usually the ones with the deepest local networks. Check hospital affiliation before premium.

Southeastern Pennsylvania

Philadelphia and its surrounding counties are a separate competitive market with their own dominant Blue plan and strong national carrier presence. Plan availability here is among the widest in the state, which generally means richer supplemental benefits and more $0 premium options than rural counties see.

Central and Northeastern Pennsylvania

Counties from Harrisburg through the Poconos and the northern tier have fewer carriers competing, and some rural counties have noticeably thinner choices. If you live in one of these areas, confirm that your current plan is still offered for 2027 rather than assuming it is, because these are the counties carriers exit first.

CMS rates every Medicare Advantage contract from 1 to 5 stars. These are the 2026 Star Ratings, the most recent published set and the ones in effect while you shop for 2027. Ratings are assigned by contract rather than by company, so a carrier’s national figure is a starting point, not an answer.

Pennsylvania follows the national Medicare calendar. There are no state-specific enrollment rules for Medicare Advantage.

  • October 1, 2026 — 2027 plan details publish for every Pennsylvania county
  • October 15 to December 7, 2026 — Annual Enrollment Period, for coverage starting January 1, 2027
  • January 1 to March 31, 2027 — Medicare Advantage Open Enrollment, one switch allowed if you are already in an Advantage plan
  • Any time — a Special Enrollment Period if you move, lose coverage, or your plan leaves your county

Step 1

Start with your doctors, not the premium

In Pennsylvania this matters more than in most states, because several plans are tied to specific hospital systems. A $0 premium plan that excludes your cardiologist costs you more than a $30 plan that includes them.

Step 2

Check your prescriptions by name and tier

Every plan publishes a formulary. A drug being covered is not the same as it being affordable, because tier placement drives your copay. With the 2027 deductible at $700, tier placement matters more than it did last year.

Step 3

Read the maximum out-of-pocket, not the premium

The out-of-pocket maximum is the number that protects you in a bad year. Two plans with identical premiums can differ by thousands of dollars on this figure, and it is the one that decides what a hospital stay actually costs you.

Step 4

Open your Annual Notice of Change

If you already have a plan, your carrier mails an ANOC by September 30 listing every change for 2027. It is the single most useful document you will receive all year, and most people throw it away unopened.

Roughly 1.6 million Pennsylvanians are enrolled in a Medicare Advantage plan, out of about 2.8 million Medicare beneficiaries statewide. That is close to 58 percent, slightly above the national average, and it makes Pennsylvania the fifth largest Medicare Advantage market in the country by enrollment.

  When do 2027 Pennsylvania plan details become available?

October 1, 2026. That is when premiums, provider networks, drug formularies, supplemental benefit amounts and out-of-pocket maximums publish for every plan in every Pennsylvania county. Enrollment opens two weeks later on October 15 and runs through December 7, for coverage beginning January 1, 2027.

  Does it matter where in Pennsylvania I live?

A great deal. Medicare Advantage plans are sold county by county, and Pennsylvania has unusually distinct regional markets. Western Pennsylvania is shaped by integrated health systems that own both hospitals and insurance plans. Southeastern Pennsylvania around Philadelphia is a separate, highly competitive market. Central and northeastern counties generally have fewer carriers.

The practical consequence is that plan recommendations you read nationally, or advice from a friend in another part of the state, may not apply to you at all. Compare what is actually offered in your county.

  What happens if my plan is discontinued for 2027?

Your carrier must send a non-renewal notice by early October. Losing your plan triggers a Special Enrollment Period, so you are not limited to the standard window, and you have until the end of February 2027 to choose a new Medicare Advantage plan.

If you do nothing, you will generally be returned to Original Medicare on January 1, without drug coverage unless you enroll in a standalone Part D plan. That is the outcome to avoid, because going without creditable drug coverage can create a permanent late enrollment penalty.

  Will Pennsylvania plans still offer dental, vision and hearing in 2027?

Many plans may continue to include supplemental benefits like dental, vision, hearing and over-the-counter allowances, though not all plans offer all benefits and the amounts vary considerably by plan and by county. Because plans are being paid more in 2027 than in 2026, there is less pressure to cut these benefits than there was in the previous two years, but nothing is confirmed until plan details publish on October 1, 2026.

  Can I switch back to Original Medicare later?

Yes. You can return to Original Medicare during the Annual Enrollment Period each fall, or during Medicare Advantage Open Enrollment between January 1 and March 31 if you are currently in an Advantage plan.

One thing to understand before you do: Pennsylvania does not guarantee your right to buy a Medicare Supplement policy after your initial six month open enrollment window has passed, outside of specific guaranteed issue situations. Leaving Medicare Advantage is straightforward. Getting a supplement to pair with Original Medicare afterwards may require medical underwriting, and you can be declined.
